Contract and Pet Owner Information

 

Terms:

All dogs are cared for daily and boarded by Top Dog Doggie Daycare, it's owners, staff, or volunteers, without liability on their part for loss or damage from disease or injury, death, running away, theft, fire, injury to humans, injury to other dogs or said dog(s), or property by said dog(s), or other unavoidable causes, due to diligence and care being exercised.  I understand that I (the dog owner) am solely financially responsible for any damage to or by my dog(s) while in the care of Top Dog Doggie Daycare. I agree that in admitting my dog(s) to Top Dog Doggie Daycare, they have relied on my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behavior toward humans or other dogs.  If my dog(s) becomes injured or ill Top Dog Doggie Daycare shall have the right to call the veterinarian as described below or the vet of Top Dog Doggie Daycare's choice within Top Dog Doggie Daycare's discretion and judgment and such expenses shall be paid promptly by me (the dog owner).

 

I have read and understand the terms set forth above and agree to abide by all terms, conditions, and statements and release Top Dog Doggie Daycare of any liabilities.
